We noticed a gap in the market whereby patients were being discharged from hospital without an affordable support system around them that would assist in allowing them to reside in the comfort of their own homes than going into care. We are a discharge enablement team, where we provide high quality care for patients who have been discharged from hospital back to their homes. We are a team of qualified nursing and care staff that provides person-centred social care services for people with various ailments. We cover mental health, individuals recovering from substance/alcohol misuse and individuals with other complex health needs, such as dementia and frailty. We are number one provider that offers this unique service and we cover the London area, through to the M25 borders. We aim to participate in and support economic growth, by helping to reduce the level of spending within the NHS, therefore giving a greater level of spend to meet the needs of patients, which translates to providing quality care in people’s homes.
